The services I received on February 18, [redacted], in Fayetteville, AR were mistakenly listed as infertility-related by Dr. Katie Beals's staff. The error has been rectified, and updated codes have been sent to QuestLabs under Z00.00. Ms. Britney from Dr. Katie's office Billing Department has already forwarded the details to QuestLabs. When I spoke to QuestLab's customer service about this issue, they mentioned that once they receive the updated lab codes classified as "General Wellness," they will work with the insurance company using the new codes, which should substantially reduce my payment amount. I kindly request to process the invoice with the corrected codes and adjust the bills accordingly. Thank you for your attention to this matter.

I would like to share a troubling experience I had at Quest Diagnostics in Doylestown, PA. The staffing situation there is concerning as there are only two women present until 10:00 AM, leaving one lady alone for the rest of the day. During my recent visit for a blood draw, the employee on duty took me and another gentleman into the back room simultaneously. She proceeded to audibly verify his personal information, compromising his privacy while I was present. This breach was repeated when she read out my personal details with him in the room. This situation is unacceptable and poses a serious privacy risk. I have decided not to return to Quest for any testing in the future. Such practices could have legal repercussions if mishandled. I was charged nearly $1,[redacted] for my entire blood work at your facility. The original doctor failed to provide my Medicare number, which covers Part A and Part B. A Medicare Service Representative advised me to update this information with you before needing to pursue an appeal. Despite not receiving a bill, I was threatened with collections through the mail. This situation has been incredibly stressful for me since last week, resulting in numerous phone calls costing approximately $[redacted] due to my pay-by-minute phone plan. I'm on a fixed income with only SSD as my source of funds, and I urgently need to resolve this issue.

I would like to correct an error in an appointment I made. I accidentally used my first name instead of my last name. I tried to fix this by creating a new appointment, resulting in having two appointments with incorrect name information. The first appointment is at 9:00 AM, and the second is at 9:30 AM. I want my original appointment to be updated with my correct name - last name Canada and first name Lillian - scheduled for Friday, September 18th, [redacted], at 9:00 AM. I also aim to cancel the 9:30 AM appointment. Furthermore, I need guidance on preparing for a Urinealysis. Should I bring a sample to the lab or provide one upon arrival? Assistance is appreciated.

As a doctor, on September 10, [redacted], my primary care physician (PCP) ordered an HbA1c test for me. However, I received results for a CBC test instead, which showed nucleated RBC, indicating potential blood cancer. This was alarming as the treatment could involve bone marrow replacement. Despite informing the lab that the incorrect test had been conducted, they confirmed those were the results. To confirm the mistake, I had to repeat the CBC test multiple times over several weeks. I reached out to a pathologist at Quest who acknowledged that the results should not have been released without proper review but did not provide a written explanation. Now, Quest is billing me for the erroneous tests, and my insurance is unaware of their errors, leading to a denial of coverage for the additional tests. I am seeking an apology and compensation for the distress caused by the incorrect results and the subsequent financial burden.

I was shopping online for a car on December 6, [redacted], and decided to check my credit score on Credit Karma. To my surprise, my score had dropped by 53 points since September [redacted], going from Excellent to now Good. Upon further inspection, I discovered an account in collections from a company I've never heard of, Transworld Systems (origin Quest Diagnostics), in the amount of $[redacted].00. After contacting Transworld Systems, they mentioned a service date of 11/25/[redacted] for a doctor visit with my former physician, Dr. Joseph Cuiffo, that was sent to them by Quest Diagnostics due to non-payment. Despite my lack of recollection of this specific visit and only going to Dr. Cuiffo's office for prescription renewals from early [redacted] to mid-[redacted] without any health coverage, I was surprised by this charge appearing out of the blue without any prior bill or notification. I attempted to contact Quest Diagnostics, but they redirected me to New Jersey due to changes in their billing system. Despite requesting a supervisor to guide me through this issue, I am yet to receive a callback. Not being aware of this bill until December 6, [redacted], I am deeply upset and demand immediate removal of this charge, or I will consider legal action. Dear Customer Service Team, I am writing to address the ongoing issue with the bill I keep receiving despite numerous previous calls. I have several recorded calls as evidence. Unfortunately, our previous conversations have been abruptly ended by your representatives. Back in October [redacted], I made a three-way call with United and Quest, where I was assured this matter would be resolved. It seems there is a lack of proper documentation on your end, as I have consistently contacted you each time I receive this bill. My history of frequent lab tests at Quest suggests a possible coding error by a Quest staff member. Shockingly, this unresolved issue has now been sent to collections, causing me undue stress and wasted time contacting both Quest and United regarding this matter. Enclosed is a recording of a call with Estaphani at Quest on 6/29/[redacted], highlighting that the test in question was never ordered by Dr. Kiltz on 6/15/[redacted]. I was notified that my account has been sent to collections, although I have been trying to set up a payment arrangement. After fully paying my account, Quest erroneously charged my Capital One Card twice. Despite Quest sending me a partial refund, half the amount remained unpaid. In June [redacted], I followed up, and a representative acknowledged the outstanding balance. As of October [redacted], the money is still owed to me. Although they claimed a check was sent and cashed in September, I have never received it. Multiple conversations with different representatives yielded no resolution. One manager denied any money was owed, contradicting previous information about an overcharge and refund. Despite assurances of a follow-up call within 48 hours, it never materialized.
